Waste substances, toxins, and water in the blood can move through the semipermeable membrane to dialysate. That's because, during blood filtration, there are diffusion and ultrafiltration events .The molecular size of blood cells and proteins is greater than waste and toxins, so it does not penetrate the semipermeable membrane. The blood that has been filtered becomes clean and is returned to the patient's body. Dialysate that becomes dirty because it contains toxic substances and garbage, then flowed out to the dialysate reservoir.
