The 4 pairs are the children of Arlan and Alviana. Seeing the nurse leave, the four twins called to their two brothers to enter the room where their mother lay on the bed.

Max, who happened to pass through Alviana's room in the care and the door was wide open, making Max panic, especially hearing his 6 children Arlan and Alviana crying very loudly.
" Mommy, Brother Arlan?" max shouted in a very loud voice.
Max walked quickly to Alviana's treatment room. His eyes were perfectly rounded to see Alviana conscious and 6 twins crying and 4 of them lying down.
Max approached the two twins to lower under the bed so as not to fall down and then carried the two twins who fell flat on their backs to stop crying.
Mommy Laras, daddy Alvonso, Alvonso, Alvian, doctors Amel and Arlan enter the treatment room Alviana wide open.
" What's up Max?" ask them in unison. They just looked at Max and the 6 twins without noticing Alviana who was conscious from her coma.
Arlan carried one of his children on the floor, as well as mommy Laras. Daddy Alvonso took one of his grandchildren in Max's hands. The second was guarded by Alvonso and Alvian.
" So Max accidentally heard the 6 twins crying because Max came here and saw Alviana's brother conscious and the 6 twins crying" said Max explained.
They simultaneously saw Alviana who had opened her eyes and approached Alviana.
" You're sober, honey? look at our 6 children" Arlan said with a happy smile.
" When I heard the voices of our 6 children crying I tried to open my eyes, they were very big fast" Alviana explained as she looked at her 6 grown children.
" You were in a coma for 2 more years Al" Arlan explained.
" What???" alviana was surprised
They distanced themselves from the doctor by still holding them.
" Sister why did this happen?" ask Arlan growled.
" I'm sorry young master, I went to the toilet and I didn't know that the 6 young twin boys went into this treatment room. Again forgive me." said the nurse crying.
" Can you work?" arlan said with a sharp gaze while refraining from yelling or hitting him considering there are 6 children.
The nurse was just crying and her legs had started to tremble in fear at Arlan's sharp gaze which was like wanting to kill her.
" It's Al, the important thing is that your wife Alviana is aware of her coma" said mommy Laras softly as she stroked Arlan's back so as to hold back her emotions.
Arlan repeatedly wasted his breath in a rude manner to dissipate his emotions.
" Max, once again thanks to you we can tell what happened" said daddy Alvonso as he hugged Max from the side as each carried the twins.
" Right Max. Thank you for helping my wife and my children" Arlan said, smiling and holding Max's shoulder.
" Yes Max, your kindness will never be forgotten" said Alvian, who remembers being shot by Clara but Max's shield left Max injured .
All smiles sincerely make Max's heart warm.
" All of you I've considered my family" Max smiled back at their smiles.
They took turns hugging Max while smiling happily. Not long after the doctor finished his work.
" How's doc?" ask Arlan
" Everything is good just nerves - Alviana nerves must be moved because it is stiff and never moved" said the doctor.
" May I move Alviana to our room, so that my wife can be close to our 6 sons and daughters?" asked Arlan somewhat doubtfully.
" May you please. Please move it to your room." said the doctor
" Oh yes doc, I happen to have a friend who can treat like Alviana to walk like usual" said mommy Laras
" Good can also be a consideration" said the doctor.
" Thank you doc tomorrow we go to my friend's clinic" said mommy Laras.
The doctor smiled and said goodbye to Alvonso's extended family.
They gathered together and laughed together and it did not feel like it was afternoon. Mommy Laras, doctors Amel and Debby and Denisa go to the kitchen.
Arlan called the butler and requested that his room be cleaned. The butler only nodded his face a sign he understood.
