Sanjaya is unaware of Ghaisan's movements, as he only focuses on Azka and Maira. And earlier Maira had deliberately turned Sanjaya's attention to Ghaisan. Ghaisan hits Sanjaya's nape and tries to retrieve the knife still held by Sanjaya.
The knife was successfully secured by Ghaisan because Ghaisan hit Sanjaya's hand, but before the knife was released from Sanjaya's hand, it turned out that the knife managed to scratch Maira's upper arm.
