Chandra's Concern


Piles of patient examination files are seen scattered on Chandra's desk. There were many things that he had to decide immediately.


Screening results show how severe the level of HIV AIDS cases suffered by Reza. A young man just entering the age of twenty-five. The productive age, where Reza should be able to reach a glorious position in his life. 


The free lifestyle of young people today makes the young man have to lie helpless. Even the rate of damage to CD4 cells that are part of white blood cells is very high. Something very difficult to control. 


On the one hand, Chandra must be able to immediately make decisions related to the young man's HIV AIDS case, on the other hand he must also immediately treat meningitis suffered by Reza.


Both need priority. 


Reza's case keeps Chandra busier than usual, and the series of follow-up tests the young man has to undergo clearly takes his time and attention. He left early in the morning, and went home almost midnight. 


This made Sonya feel a little neglected, for her Chandra seemed to be moving away. 


"You haven't slept?" Chandra enters his bedroom with Sonya, and sees his wife changing television channels. 


"How can I sleep if my husband hasn't come home?" Sonya replied in passing. 


"Wait a minute, I'll take a shower." Chandra went into the bathroom without looking back. Sonya who saw the cold attitude of her husband just sneered. 


After thirty minutes passed, Chandra was finally tidy in his sleeping pajamas. Then he walked up to the bed and sat down next to his wife. 


"Have you eaten?" Chandra hopes to ease some of the tension between them. 


"No need to make small talk. Every day, you go to work before the chickens crow, and come home when the owls start coming out of the nest. You think this is a hotel?" Sonya Sulu. 


Chandra smiled warmly at Sonya's words. He realized, lately he was more busy and more often in the hospital than at home.


"I'm sorry, you must have heard the case of meningitis patients as well as HIV AIDS patients?" 


"Yes, I know. But there are a lot of doctors in the hospital, not just you. Why does everything have to be done by yourself?" Sonya started to get fiery. 


"Yes, there are indeed many doctors with brilliant achievements in the hospital, but still, I have my own responsibility there. I can't just hands off Sonya, '" Chandra explained carefully. 


"Are you so busy you don't have time for me Chandra?" Sonya's face this time looks so clear. 


Chandra was staring at Sonya's question. A simple and commonplace question for married couples, if it is a normal couple. But not with them, Chandra thought. 


"All right, I'm sorry. I promise, once there is a common ground in this case, I will definitely spend more time for you," Chandra said softly. He was eager to caress Sonya's head, and remove the child's hair covering her forehead, but he was afraid Sonya became uncomfortable and made the condition of their household deteriorate.

"Doctor, this is Reza's patient's follow-up." Lisa placed a fairly thick map in front of Chandra. 


"Thank you so much Lisa. What about the psychological state of the patient and his parents?" Chandra should ensure that the patient and his family are breastfed, as it is one of the important factors in treatment. 


"The patient's parents are still devastated, but they are seen trying to toughen up in front of their child. The patient does not seem to know the true condition. Why didn't the doctor tell me the truth?" 


"Remember Lisa, not only is she an ODHA, she also has meningitis. His current condition clearly did not allow to think of anything heavy, it would not be good." Chandra replied while reading the results of the examination. 


"The amount of RNA above one hundred thousand copies per milliliter of blood, obviously cannot be handled easily. Plus the case of meningitis that demands optimal body resistance. Antiretroviral treatment (ARV) also can not all be used, only three types. Since he was immune to the other two," Chandra let out a frustrated sigh. 


"Lisa, please pass this report to the internal medicine section. Please tell the other doctors too, I want everyone involved in this case to gather in the boardroom within the next three hours. We have to take action immediately" Chandra knows, he needs another opinion and support. 


All the doctors involved in Reza's case had gathered in the meeting room at Chandra's request. They all felt confused as well as pleased, as a main physician still valued their opinions. 


"Sorry, I'm late. There's some files I need to check first." Chandra sat behind the table in authority.


 


Everyone who was there nodded, in a sign of accepting Chandra's apology. They were obviously surprised by what Chandra did, as the man's standing was clearly higher. But he did not mind apologizing for his mistake. 


"All right, just go ahead. You guys must have guessed why I asked all of us to get together all of a sudden. We are running out of time, meningitis suffered by patients is getting heavier. While the patient's body endurance is also getting weaker. At least there's an ARV drug coming in tonight. But meningitis drugs may not react if they collide with the ARV content. Can any of us give me another solution?" Chandra looked at their faces one by one. 


"I think we should try the slightest possibility Chandra. Because it could be that small possibility of becoming big." A doctor named Tommy speaks. 


"Tommy's right Chandra, put the ARV in and we'll see what the reaction is. As long as there was no overreaction, it meant that the two drugs could go hand in hand, "this time a senior doctor expressed his opinion. 


"The problem here, there are three types of ARV that can be accepted by the patient's body. That means I have to insert one by one of the three ARVs, to see the reaction of the meningitis drug. It definitely takes time, whereas as I said earlier at the beginning. We're running out of time." Chandra answered with a sharp look. 


The four people other than Chandra who were in the room whispered to each other hearing an explanation from him. Before they entered the meeting room, all agreed that they would use this case as a tool to bring Chandra down. They clearly could not accept if Chandra, who was only a lucky nurse in their eyes, could hold the post of primary physician. It was clearly an insult.
